Within international development [1], public health [2], and clinical medicine [3]–[5], there is increasing interest in determining whether cash payments or other economic incentives can be used to influence the choices and behavior of individuals and groups in order to promote desired health goals. However, a number of complex issues affect the review and approval by research ethics committees of research studying the effectiveness of using financial incentives to promote desired health goals. Current ethical and regulatory frameworks regard the provision (...) of gifts or cash payments to participants in human research as potentially problematic. Specifically, these frameworks imply that such incentives may undermine the autonomy of participant choice, hinder the disclosure of medical information, exacerbate social inequalities, or result in the exploitation or degradation of vulnerable populations [6]–[9]. Typically, these frameworks provide guidance about payment to research participants to reimburse expenses, to compensate for time and effort, to provide insurance coverage, or as an incentive to participate in the research itself. However, the issue of payment as a component of the research intervention is relatively new. RECs thus lack explicit guidance about ethical issues surrounding research that evaluates the use of financial incentives as an intervention to promote health. (shrink)

The extant clinical literature indicates profound problems in the assessment, monitoring, and documentation of care in long-term care facilities. The lack of adequate resources to accommodate higher staff-to-resident ratios adds additional urgency to the goal of identifying more costeffective mechanisms to provide care oversight. The ever expanding array of electronic monitoring technologies in the clinical research arena demands a conceptual and pragmatic framework for the resolution of ethical tensions inherent in the use of such innovative tools. CareMedia is a project (...) that explores the utility of video, audio and sensor technologies as a continuous real-time assessment and outcomes measurement tool. In this paper, the authors describe the seminal ethical challenges encountered during the implementation phase of this project, namely privacy and confidentiality protection, and the strategies employed to resolve the ethical tensions by applying principles of the interest theory of rights. BackgroundThere is increasing need for peer reviewers as the scientific literature grows. Formal education in biostatistics and research methodology during residency training is lacking. In this pilot study, we addressed these issues by evaluating a novel method of teaching residents about biostatistics and research methodology using peer review of standardized manuscripts. We hypothesized that mentored peer review would improve resident knowledge and perception of these concepts more than non-mentored peer review, while improving review quality.MethodsA partially blinded, randomized, controlled multi-center study (...) was performed. Seventy-eight neurology residents from nine US neurology programs were randomized to receive mentoring from a local faculty member or not. Within a year, residents reviewed a baseline manuscript and four subsequent manuscripts, all with introduced errors designed to teach fundamental review concepts. In the mentored group, mentors discussed completed reviews with residents. Primary outcome measure was change in knowledge score between pre- and post-tests, measuring epidemiology and biostatistics knowledge. Secondary outcome measures included level of confidence in the use and interpretation of statistical concepts before and after intervention, and RQI score for baseline and final manuscripts.ResultsSixty-four residents (82%) completed initial review with gradual decline in completion on subsequent reviews. Change in primary outcome, the difference between pre- and post-test knowledge scores, did not differ between mentored (−8.5%) and non-mentored (−13.9%) residents (p = 0.48). Significant differences in secondary outcomes (using 5-point Likert scale, 5 = strongly agree) included mentored residents reporting enhanced understanding of research methodology (3.69 vs 2.61; p = 0.001), understanding of manuscripts (3.73 vs 2.87; p = 0.006), and application of study results to clinical practice (3.65 vs 2.78; p = 0.005) compared to non-mentored residents. There was no difference between groups in level of interest in peer review (3.00 vs 3.09; p = 0.72) or the quality of manuscript review assessed by the Review Quality Instrument (RQI) (3.25 vs 3.06; p = 0.50).ConclusionsWe used mentored peer review of standardized manuscripts to teach biostatistics and research methodology and introduce the peer review process to residents. Though knowledge level did not change, mentored residents had enhanced perception in their abilities to understand research methodology and manuscripts and apply study results to clinical practice. Counterpunch, November 23, 2009 In his wild and slanderous "Open Letter to Amnesty International" (signed, fittingly, "Yours, in disgust and despair"),[2] The Guardian - Observer's veteran reporter Ed Vulliamy explains that two "main concerns" motivated him to draft his repudiation of AI's choice of Noam Chomsky to deliver this 2009 Stand Up For Justice lecture: One is that the "pain" individuals such as Chomsky are alleged to cause the "survivors and the bereaved" of the wars in the former Yugoslavia is (...) "immeasurable," and Vulliamy feels some kind of need to help mitigate this pain; the other, apparently, is that the "historical record" as it pertains to these wars is too precious and too fragile to be left in the wrong pair of hands. "For Amnesty International, of all people, to honour this man is to tear up whatever credibility they have estimably and admirably won over the decades, and to reduce all they say hitherto to didactic nonsense," Vulliamy writes. "By inviting Chomsky to give this lecture, Amnesty condemns itself to ridicule at best, hurtful malice at worst -- Amnesty joins the revisionists in spitting on the graves of the dead.". (shrink)

Addiction Trajectories is a collection of anthropological essays that brings a refreshingly human perspective to the scientific pursuit of addiction. This book encourages the reader to step back from the details, giving voice to the experiences of the drug user as they grapple to come to terms with their condition and the efforts of the treatment community. At the same time, the book provides insight into the machinations of the treatment community struggling to understand the scope of their task and (...) to respond to the shifting intellectual landscapes of licit and illicit drugs. The addiction trajectories framework is constructed around three themes. Theme one concerns the changing categories and concepts of addiction through time and across disciplinary boundaries. Theme two represents changes in therapeutic concepts of addiction across cultures and organisations. Theme three represents the experiences of the addicted individual. In this review, I argue that Erik Olin Wright’s Envisioning Real Utopias is necessary reading for anyone interested in thinking through the possibilities of creating noncapitalist ways of organising economic and social life in the world today. However, I also raise questions about Wright’s deterministic interpretation of Marx’s critique of political economy, his relative neglect of class-analysis, and his non-Gramscian conception of the relationship between the state, economy, and civil society.

That picture will not do. Ifthe painter were to cease painting, what he has made up to that point would remain in existence, whereas it is precisely the ability to stand on its own that the doctrine of continuous creation seeks to deny of what is made. Nor does anything in the relation of artisan to artifact properly convey the special intimacy and presence that the act of continuous creation is supposed to establish between God and creatures. A painting or a story does not "live and move and have its being" in the one who fashions it-nor would even a living and thinking creature, if the manner of its making were solely that of an artisan. These shortcomings raise the question of whether we should seek to replace the image of artisan and artifact with one that is more dynamic. We might think of God as like a singer and of what is made as His song, or of God as like a dancer and of what is made as His dance. By envisioning the act of creation as a "doing" rather than a "making," analogies of this type more adequately indicate the continuing ontological dependence ofcreation on Creator. They also draw our attention to a mode of presence radically different from that of one body to another, the presence of an agent "within" his own actions. They may thus bring us closer to what St. Paul presumably has in mind in the passage quoted from Acts. On the other hand, they have faults of their own, for they are hard to reconcile with creaturely freedom and the reality of secondary causation. How could any kind of "doing" act contrary to the will of its doer? Indeed, how could it act at all? Actions are not causal agents; a song does not act in its own right, but is at best an instrument of the one who sings it. So it would seem that, taken to their logical conclusion, analogies such as that of the singer and his song lead to the denial of both creatures' freedom and their causal efficacy. David Bohm, Emeritus Professor of Theoretical Physics at Birkbeck College of the University of London and Fellow of the Royal Society, died of a heart attack on October 29, 1992 at the age of 74. Professor Bohm had been one of the world’s leading authorities on quantum theory and its interpretation for more than four decades. His contributions have been critical to all aspects of the field. He also made seminal contributions to plasma physics. His name appears prominently (...) in the modern physics literature, through the Aharonov- Bohm effect , the Bohm-EPR experiment , the Bohm-Pines collective description of particle interactions (random phase approximation), Bohm diffusion and the Bohm causal interpretation of quantum mechanics, also sometimes called the de Broglie-Bohm pilot wave theory. David Bohm was born in Wilkes-Barre, Pennsylvania on December 20, 1917. A student of J. Robert Oppenheimer, Bohm received his Ph.D. from the University of California at Berkeley in 1943. In 1950 he completed the first of his six books, Quantum Theory, which became the definitive exposition of the orthodox (Copenhagen) interpretation of quantum mechanics. Here Bohm presented his reformulation of the paradox of Einstein, Podolsky, and Rosen. It is this Bohm version of EPR which has provided the basis for the enormous expansion of research on the foundations of quantum theory, focusing on nonlocality and the possible incompleteness of the quantum description (the question of “hidden variables”), which has occurred during the past several decades.
